Forms explained

You can submit a renewal form online or mail back the form/s supplied to you with your annual fee notice. Your online form is available once you have received your annual fee notice (at least two months prior to your registration end date).

Renewal or Restoration of Full Registration Form

A renewal application kit is provided in the envelope with your fee notice at the end of your five year period of full registration.  This kit contains a printed Renewal or Restoration of Full Registration Form, Renewal or Restoration Declaration Form and Guidelines for Application for Renewal or Restoration of Full Registration.  If you decide not to use the online form you can fill in and post your renewal form/s in the reply paid envelope provided.

Renewal or Restoration Declaration Form

If you decide to submit a paper form rather than filling in and submitting an online form you will only need to complete the Renewal or Restoration Declaration Form if you answer YES to a question in Section 4 – Suitability to Teach on the Renewal or Restoration of Full Registration Form.

Applying via your online services account

The online Renewal or Restoration of Full Registration Form is accessed via your QCT Online Services account once you receive your fee notice near the end of your five year period of full registration. Only one online form is required to be submitted as the form includes the renewal declaration if it is applicable as a result of your answers in the form. You can also elect to pay your fee online at the end of completing your online form. Your form is then submitted online - there is no need to print it out or mail it to the QCT.

Application for restoration

To apply for restoration of your teacher registration, within three months immediately after your registration end date, you must use the same form/s as you would for renewal and pay the appropriate fees (including the restoration fee).
